COVID-19 Vaccines

Ephraim McDowell Health is currently scheduling appointments to provide the COVID-19 vaccine to individuals age 18 and older based upon the state’s vaccination plan.

Appointments may be scheduled by calling (859) 936-8350 Monday through Friday from 9 a.m. to 5 p.m. Walk-ins are also accepted Wednesday through Friday between 8 a.m. and 4 p.m. The Vaccine Clinic will be closed Thursday, April 22, and Tuesday, April 27.

How to join the waitlist

To put your name on a waiting list for the COVID-19 vaccine, we are providing two options:

(Monday – Friday from 9 a.m. to 5 p.m.)

You must bring identification with you.

If you are a teacher or healthcare worker, you must show proof of employment.

Once you are scheduled

Vaccines will be provided at the Boyle County Extension Office, located at 99 Corporate Drive off Lebanon Road.  Parking is available behind the building.

If you need transportation to our vaccine clinic, please call Blue Grass Community Action Partnership at (800) 456-6571. This applies only to residents in Boyle, Casey, Garrard, Lincoln, Mercer and Washington counties.

If you are able, please print and complete the following consent forms and bring them with you to your scheduled vaccination appointment.

COVID-19 Vaccine Consent

Please note that a limited number of vaccines are available.

Vaccination Center – State of Kentucky Mandates:
On February 1, 2021, the COVID-19 vaccination priority will be Phase 1B, persons age 70 years or older.  All Vaccination Sites are asked to prioritize this population until further notice.  Other persons from Phase 1A and 1B remain eligible for vaccination.  As vaccine quantities and available appointment times allow, persons in Phase 1C may also be scheduled to ensure each vaccination site administers 90% or more of all vaccine doses received within 7 days of arrival.
