If you are hospitalized for at least three days because of an illness or injury, you may need transitional care services. This is when you reach a point in your recovery where you can leave the hospital but you aren’t able to care for yourself at home. You might need skilled care or some other type of continuous treatment that requires the services of a registered nurse around-the-clock and may include extensive rehabilitation.

We offer three transitional care options to help patients ease between their hospital stay and being discharged home. Case managers will work closely with you and your family to help select the best option for you, based on payment sources and services that are necessary for your optimal recovery.

Swing Bed

We offer Swing Beds at Ephraim McDowell Fort Logan Hospital and Ephraim McDowell James B. Haggin Hospital that provide short-term, temporary placement for patients who require a skilled level of nursing care but are not yet ready to be discharged from acute care to home. Services may include IV therapy, pain management, palliative care, wound, physical therapy, occupational therapy or speech therapy. If approved for admission, services are covered by Medicare, Medicare HMO, Veterans Affairs and most private insurance companies that cover skilled care services.

Assisted Living, McDowell Place of Danville

McDowell Place of Danville features 98 apartments that offer independent living, assisted living, personal care as well as respite care services. Assisted living provides assistance with activities of daily living including self-administration of medications while personal care provides primary care by a nurse including medication administration. Respite care is available for short-term stays to recuperate from a recent hospital stay, surgery or fall. Palliative care is also available.

Residents may contract with an outside home health agency for physical therapy, occupational therapy or speech therapy as well as IV therapy, skilled treatments for wound care and respiratory therapy. McDowell Place accepts residents who self-pay for their stay. Some long-term care insurances are accepted as is Veterans Affairs benefits.
